Description
One pathway for physicians in the QPP is participation in Alternative Payment Models which seek to further improve value in healthcare and to return some of the financial savings back to physicians. During this session, we will discuss the RPA-designed Incident Dialysis Alternative Payment Model focusing on the optimal transition to dialysis, including pre-emptive transplantation, modality choice, advanced care planning and care of the incident dialysis patient.
